Our process begins with a health care professional coming out to your home to simply listen to what you want from a home care provider. At this meeting we will take a comprehensive assessment in order to generate a personal care plan. After evaluating the assessment we will then create a detailed plan of care to present to you. Once this process is complete, CMK will select a certified and bonded home health aide that best matches your requirements.
When families require assistance with understanding the complex aspects of the healthcare system, CMK provides case management services to alleviate the stress of such things as determining insurance eligibility, locating participating providers, and attending doctor appointments.
