Home Care Aides
Works under the supervision of the designated registered nurse will provide the direct client care as assigned. Also, they will assist in the home care services that reflect CMK Home Care’s mission and standards.
Essentials Duties and Responsibilities
- Non-medical in home care and assistance for seniors and disabled adults and children
- Assist clients with personal care such as bathing, dressing, toileting, and transferring
- Assist clients with transportation, errands, laundry, light housekeeping and meal preparation
- Provide companionship
- Socialization and emotional support
- Maintain a pleasant and safe environment
- Plan and prepare meals and shop for client
- Takes temperature, pulse, blood pressure, and respiration
- Medication reminders
Requirements
Must have the ability to work per diem schedules including weekday, weekend, morning, evening and overnight shifts.
Experience
A minimum of 1 year verifiable experience providing personal care assistance. High school diploma or general education (GED) preferred; completion of an approved training program in Home Health Aide services or completion of an approved competency evaluation program required.
Registered Nurses
R.N.’s are responsible for the delivery of patient care services through implementation, and supervision. They will also participate in quality improvement activities to comply with CMK Home Care as well as state and federal guidelines.
Essential duties and responsibility
- Intake and coordination of a referral form and acceptable referral source
- Initial in home assessment
- Establish the patient’s individual treatment and client record baseline.
- Collaborate with other personnel and contract services to coordinate and implement the plan of care.
- Supervises the implementation of the plan of care and the quality of care delivered to the patient.
- Provides in home supervision of Home Health Aide Services at least every 30 days for patient receiving aide services and documents the visit in the client record.
- Provides supervision of CNA’s assigned to patient caseload, through directing the implementation and evaluating the effectiveness of the nursing care and patient’s plan of treatment.
- Provides client and staff supervision 24 hours a day when assigned on-call.
- Collaborating with leadership/management team and office staff to facilitate timely office processing of paperwork and personnel information.
- Actively participates in CMK Home Care quality improvement activities.
- Promote CMK Home Care and services to the public; participate in physician and community education related to services.
- Performs other duties as assigned.
- Maintains responsibility to develop professional self attend seminars, and identify self learning needs, collaborates with agency leaders in meeting learning needs.

Must be a registered nurse with at least 1 year experience with public health, home health care, or hospital background
